Graduate Affairs Electronic Forms

Graduate Affairs transitioned from paper-based forms to automatically-routed electronic forms (e-forms) in Spring 2019. The forms are currently available to all Master's, Doctoral, and advanced certificate program students. 

OnBase is our system for electronic forms and workflow. This process has been designed to replicate the paper-based process as much as possible. 

Once you have activated your account, you will receive notifications when a new form is available for your review. You will also receive twice weekly reminders when there are forms requiring your attention. After a form has been in your queue for 14 days, your department chair (or equivalent) will be added as an alternative approver in order to minimize delays in processing.

Log In and Guides

 Log in to OnBase using your SDSU ID and password.

The Graduate Advisor user guide provides an overview and instructions on how to use OnBase to submit, process, and view e-forms.

The student user guide provides instructions on how to submit an electronic form.
